You are invited to submit an abstract to be considered for presentation at
the 14th International Forum on Quality and Safety in Health Care which
will take place from 17-20 March 2009 in Berlin, Germany.

The International Forum is open to all, and is relevant to both beginners
and those experienced in improvement work. This year, we aim to be more
inclusive, and to attract previously unrepresented organisations and
countries. We particularly encourage submissions from those who may be
first time submitters, have previously submitted unsuccessful abstracts, or
who are new to the quality/safety field. Detailed instructions for
